Building Forms and Surveys for Lead Capture

Form and Survey Overview: #

Forms and surveys in AiXios are essential for capturing leads and information from prospects. Forms are typically single-page contact forms, while Surveys can be multi-step forms or questionnaires with conditional logic.

Build a Lead Capture Form: #

In the sub-account, navigate to Sites > Forms and click + Create New Form to start a new form. An empty form canvas will appear with a toolbox of fields. Drag and drop the fields you need onto the form canvas. For a simple lead form, you might drag Full Name, Email, and Phone. Arrange the fields in your desired order by dragging them. Click on any field to edit its settings (mark Email and Phone as Required). Add a Submit Button if it’s not already on the form. Click Options or Settings for the form to configure what happens on submission: set a Thank You Message or a Redirect URL. Once done, click Save and give the form a name.

Design a Multi-Step Survey: #

Go to Sites > Surveys to open the Survey Builder and click + Create New Survey. Build your survey by dragging fields/questions onto the canvas. Use the Add Page or Add Slide button to create additional steps in the survey. For each field/question, you can edit its options. For multiple choice, define the answer options. If you want conditional branching, you can set that up in the Logic settings. Click the Styles tab to customize appearance and add a progress bar. Under Options, set the survey name and the On Submit action. Save the survey and name it.

Embed or Deploy Forms and Surveys: #

To use the form on your website or funnel, go to Sites > Forms, find your form, and click Integrate Form. Here you’ll see options: a direct link to the form and an embed code to embed it on an external site. If you built a funnel or landing page in AiXios, open your funnel page in the editor, and drag a Form element onto the page. In the settings on the left, select your saved form from the dropdown. For surveys, similarly click Integrate Survey to get a survey link and embed code.
